Purchase a brick to help build the Peoria Dog Park at Bradley Park. Your brick can be in "celebration" of your pet, "in memory" of a deceased pet, or contain a message such as, "spay and neuter". You may also advertise your business on a brick.

• Each brick may have up to three lines of copy.

• Purchase of a brick is a tax deduction to the extent allowed by law as this is a fundraiser.

• Cost of each brick is $50 until May 31, 2008. Beginning June 1, 2008, the cost of each brick will be $75 each.

• To purchase a brick, download this form, complete and mail with your check made out to Peoria Park Foundation with a note in the message area the reads "for Dog Park".
