HOW TO
REGISTER FOR CLASSES
NOTE: At
this time, on-line registration and
phone-in registration are NOT available.
However, you may print a form and register one of 4 easy ways:

#1. Mail In - April 21 through the season
Complete registration form (print
one out) and return to us.
Confirmation of class registration will NOT be sent unless a self- addressed
STAMPED envelope is enclosed.

Complete registration form. Credit card (Visa, MC or Discover) payment
only. When registering by fax, it is understood that the facsimile document
received (including the waiver and release of claims) shall have the
same legal effect as the original form.

Come in to the Glen Oak Park Pavilion Recreation Office, 2218 N.
Prospect Road in Glen Oak Park:
M-F
April 21 - May 23* 8:00 am -
6:00 pm
Tu-F May 27 - May 30
8:00 am - 5:00 pm
M-F May 26 - season 8:00 am-4:30 pm
*The office will be closed 5/26,
7/4 and 9/1.
#4. After Hours
Drop Off
For your convenience, complete registration form and use the night
drop box on the Recreation Office door. The office is located in Glen
Oak Park (Prospect and McClure) in the large pavilion at the back of
the park.
